Did you know?
How Diabetes affects people at home
Living with diabetes at home often means making routine changes. Taking medications on time, checking blood sugar levels, planning meals, fitting in exercise and being alert for signs of complications. Everyday tasks like getting up, walking, preparing food or attending appointments can feel more demanding when managing fatigue.
For families and loved-ones it means stepping into a supportive role more frequently – helping coordinate appointments, understanding dietary changes, offering encouragement and adapting the home environment. While many people manage well, the unpredictable nature of the condition (for example sudden changes in glucose levels) can bring worry, especially for those living alone. Having reassurance and consistent help can turn the focus from surviving the condition to living with it confidently.

What is Diabetes?
Diabetes is a long-term condition where the body either cannot produce enough insulin, or cannot use insulin effectively, leading to elevated blood glucose levels.
There are two main types: Type 1 (where the body cannot make insulin) and Type 2 (where the body does not use insulin well or does not produce enough). Without proper management the condition can lead to damage of te blood vessels, nerves and organs.
How we support people with Diabetes

At Horizons Homecare our approach is tailored to the person and the home rather than the task. We listen to the individual and their family to understand their daily routines, preferences and lifestyle. Our carers assist with timely medication reminders, help monitor wellbeing and encourage safe movement and hydration. We support healthy routines, nutritional choices and gentle activity that suit the person’s pace. With consent we liaise with health professionals, translate medical guidance into everyday life and keep families informed so they feel part of the care journey. Our aim is to make the home a place of comfort, continuity and independence for people managing diabetes, supported by skilled care that adapts as needs evolve.
Insulin Administration Training (if required)
We can train our staff to administer insulin where required...
Horizons Homecare ensures that clients who require insulin injections receive safe and competent support from trained care professionals. While our care staff do not prescribe or alter medication, they can be trained by accredited external healthcare trainers to administer insulin safely in line with the client’s individual care plan and clinical guidance.
We work with trusted, approved training providers to deliver specialist insulin administration courses that meet current CQC and NHS best practice standards. This ensures that only staff who have completed this training and been assessed as competent will provide this level of support.

Our core values...
The foundation of our success
Kindness
We believe that being truly kind isn't a skill, and it certainly isn't something you can learn. Being kind by nature is who you are!
Thats why we only employ, train and nurture those who are truly kind and caring towards other people. Not only does this belief form our recruitment process and decisions, it has become one of the foundations of our business.
Punctuality
We firmly believe that routines are what keep us as people, independent. Why should that change just because you require support at home?
Punctuality is another foundation of our business, and it means that when you become a client of Horizons, you will always know when your care team will be arriving. No more care workers being late, and no more missed visits.
Continuity
Something we have always been very eager to improve is the quite possibly the biggest 'flaw' in social care services today. Continuity...
After all, we wouldn't want our loved ones seeing a different person at every care visit, so why should you have to put up with it?
This is why you will have your very own team of care workers that really understand you and your routines. Your history, your family and your hobbies.
Family
The word family, encapsulates everything that we stand for, but unlike others who say they are a family run company. We truly are!
From our HCAs to our clients, we are all one family striving for better outcomes and happier lives. This is why family is not only a core value, but the bricks and mortar we build the foundations on.
